Home Health Assessments
Hazelwood Clinic offers a service to our patients who are 75 years and older in which a nurse, employed by the clinic, will visit you at your home to do a thorough medical assessment of your needs in your home environment. If you have any concerns about the calls, please feel free to verify with the our friendly reception staff.
The service is entirely Government funded and is a great opportunity for our nurses to assess your home environment and any other needs you might have in order to assist you to continue to stay safely in your own home. They may even be able to suggest added supports available to you.
We understand that many of our 75 + year old patients are very fit and well and many of you feel you do not have the need, but this is the best time because they see you at your best and with subsequent visits are trained to detect any change over the years and alert your regular GP to these changes. Supports can then be suggested to be put into place so that living at home remains safe for you.
What is included in a Home Health Assessment?
One of Hazelwood Clinic’s friendly Registered Nurses will visit you at home and check:
- Recent and current health
- Review of Medications
- Living arrangements and home safety
- Mobility
- Social support
- Nutrition and exercise
- Any difficulties with mood, memory or sleep
- Blood pressure, pulse, weight and urine test.
Our nurse will then arrange an appointment for you with your regular GP to follow up from your home visit. In this appointment your GP, with your input, will be able to advise you of any recommendations based on our nurse’s information to help you maintain or improve your health and home environment with the goal of assisting you to stay in your home for as long as possible.
